This Release & Waiver of Liability (“Release”) releases Mobilty Made Fun, Stairlifts Made Simple, Cucamunga Medical, Omeo Group Limited, Omeo Technology Limited, Omeo America, AGM Container Controls and its and their officers, directors, shareholders, members, managers, owners, employees, insurers, attorneys, sales agents, and retail partners (collectively “Releasees”) from any claims, causes of action, suits, actions damages of whatsoever kind or nature, and demands (each a “Claim” and collectively “Claims”) arising under or relating to personal injuries and/or property damage, caused by an Omeo trial and/or demo.

By signing this Release, the undersigned, for themself and or their heirs, estates, executors, administrators, successors, and assigns:

• Acknowledges they have been informed of the risks involved in riding and/or using the Omeo. Those risks include but are not limited to losing control of the Omeo, accidentally injuring themselves and others, and damaging property.

• Irrevocably agrees to release the Releasees, individually and collectively, from any liability, including financial responsibility, for any Claims and knowingly waives any rights to assert a Claim against any Releasees for any reason.

• Attests they have informed Omeo trainers of any medical conditions, functional and cognitive limitations, or impairments to determine if they are suitable to use the Omeo. They do not have any conditions beyond those disclosed to Omeo representatives that will increase the likelihood of injuries to themselves or others or damaging property while using the Omeo.

• Agrees to obey all safety precautions as listed in writing and as verbally explained to them. When needed, ask for clarification on the instructions, safety precautions, and safe use. Further, they agree to take every precaution to ensure they, any other user of the Omeo, or any other party suffers no personal injury or damage directly resulting from using the Omeo. It was designed and made available to use the Omeo for this purpose.

Once signed by you, this Release releases your legal right to take any legal, administrative, or other action against Releasees for any claims you may have against the Releasees in connection with the activities set out above.
